mitral valve and rheumatic fever

When I was 7 I had rheumatic fever that had gone untreated for awhile. I am now 57 and in Feb I had to go to the ER for chest pain, had a heart cath done and have a 50% blockage in left main artey, also mild mital regurg and 2+ tricuspid leak, and slighly enlarged left atrium,also the mitral leaflets are thick. Have hypertension also, I am thin, don't smoke and try to eat right. Due to the rheumatic fever, does anyone know if it would be time to replace the valve?
