Have you heard of Myalgic Encephalomyelitis? It's concurrent with many of the physical and psychological symptoms you're describing.
BUT you need a proper diagnosis. Start with a good GP who will listen to your descriptions of your symptoms and give you a thorough examination, and referred tests if necessary, to rule out obvious problems (there'll be nothing more invasive than a blood test, and you'll probably need to be referred for an ECG to check your heart, unless your doctor has a machine). If nothing conclusive is identified, ask to be referred to someone specialising/experienced in ME.
Also at 28, you're way too young to be signed off with anything! There are all sorts of treatments for musculo-skeletal and orthopaedic problems, but once again you need a sympathetic and broad minded GP.
You wrote to the heart and cardiology forum, but it's pretty unlikely there's anything wrong with your heart from what you describe. BUT any recurrent palpitations and breathing problems should always be evaluated.
You should start today to get the help that you need to get that young life of yours back on track.
